Taylor Swift's Patient Zero Video Twist Explained

The singer's self-directed short casts her as a murdered first wife haunting Dakota Johnson's character, with Colin Farrell playing the husband responsible for both marriages.

Forget the red carpet moment for a second. The real story in Taylor Swift's "Patient Zero" video is the twist buried inside it, and it's a nasty one.

Swift debuted the short during Sunday's MTV VMAs, and unlike most pop videos built around performance shots, this one plays out almost like a horror short film. Swift plays the ghost of Colin Farrell's character's first wife. Dakota Johnson plays his current one. For most of the runtime, Swift's character shadows Johnson through her days, invisible, watching her get sick, watching her fight with Farrell, watching her slowly come undone.

The two women open the video standing at a graveyard in matching gray outfits, which reads like a throwaway image until the ending recontextualises it. That grave belongs to Swift's character. Farrell poisoned her. The marriage Johnson is currently living through is a repeat.

The fight scenes escalate from there. At a gala, Johnson's character finally sees the ghost and collapses. In the car afterward, Farrell turns on her, accuses her of humiliating him, tells her he thought she was different. "Different from who?" she asks. Cue the flashback that answers the question nobody wanted answered.

Cara Delevingne shows up too, playing a woman Farrell's character flirts with mid-marriage, which only tightens the pattern the video is drawing around him.

Swift directed the whole thing herself and brought in Emmanuel Lubezki, the Oscar-winning cinematographer behind films like "Gravity" and "Birdman," to shoot it. That's not a small credit to attach to a music video, and it's part of why MTV handed her its first-ever Artist Director Honor at this year's ceremony, recognising work behind the camera rather than in front of it. Johnson presented the honor before the two watched the video together from the crowd.

Johnson, at the London premiere of "Verity" last Thursday, called the shoot "the most fun thing on the planet," adding that working with people you already trust changes how the whole process feels.

The timing wasn't random either. Swift had spent weeks scattering hints, in the "Opalite" video, on social media, even in a pre-recorded "Law & Order: SVU" cameo at the Emmys. Tuesday brought the "Patient Zero" single announcement. Wednesday brought the bigger reveal, that the track is part of "The Life of a Showgirl: The Encore," a four-song extension of last year's album, alongside "Cleveland!," "Pink Clouding" and "Babylon." All four went up for streaming Friday.

What's left unclear is whether the story Swift built for "Patient Zero" connects to anything else on the expanded album, or whether it's a standalone piece she wanted Lubezki's camera on. Given how deliberately she's been dropping breadcrumbs all year, it would be surprising if this is where the trail ends.
